Rebecca RainesRebecca Raines, RN is the President and founder of Transitional HealthCare Solutions, LLC, providing Care Transition  and Care Management services to the general public, medical organizations, medical facilities and self-insured employers  throughout Northeast Ohio. 

A goal of Transitional HealthCare Solutions, LLC is to provide a customized service that fits the needs of the individual and their caregiver.   

The Care Transition program empowers chronically ill individuals and/or their caregivers to take a more informed and active role in their healthcare.    However, not everyone can take an active role in their healthcare or even their everyday activities of daily living.  For this reason, Care Management services were added to our service solutions.

On May 14, 2010, Transitional HealthCare Solutions became a Limited Liability Company and adopted the Care Transitions ProgramSM,  an evidenced based model created by Dr. Eric A. Coleman, M.D., M.P.H., University of Colorado.   On July 28, 2010 Rebecca Raines, RN became a Transition CoachTM through training directed by Dr. Eric Coleman and his team of coaching professionals.

Helping individuals navigate the complex and overwhelming healthcare system in order to achieve  higher quality services and more efficient care has been a passion of Rebecca’s nursing career.

Rebecca received her registered nursing education from Valencia Community College in Orlando, Florida graduating in August 1994.  In addition to her Transition CoachTM credentials, she is also an Associate Care Manager and member of the National Association of Professional Geriatric Care Managers.  Rebecca will obtain her Certification in Care Management in 2012. Throughout her 17 years of nursing,  Rebecca’s experiences has lead her in areas of geriatrics, medical/surgical, neonatal intensive care, home health care, critical care resource specialty (PICU, NICU, burns), correctional nursing, MRDD, and long term acute care.  Her many roles in these areas have consisted of Staff Nursing, Care Management,  Health Services Administrator, and Director of Nursing.
